目的

探讨甲钴胺联合鼠神经生长因子(NGF)对外伤性四肢周围神经损伤的临床研究效果。

方法

100例研究对象均来自2012年1月至2015年1月在广西百色市人民医院骨科住院治疗的四肢周围神经损伤患者,均排除伤前患有其他神经系统疾病。将100例周围神经损伤患者利用简单随机化分组的方法分为治疗组(甲钴胺联合NGF组,50例)和对照组(甲钴胺组,50例),分别于治疗1月、3月后比较两组患者疼痛、麻木症状改善程度及神经电生理变化的改善情况。

结果

与对照组比较,治疗组患者的疼痛、感觉麻木症状显著减轻,对照组和治疗组的疼痛症状有效率分别为56%(28/50)、94%(47/50),两组比较,差异有统计学意义(χ2=19.25,P<0.01);感觉麻木症状有效率分别为64%(32/50)、88%(44/50),两组比较差异有统计学意义(χ2=19.95,P<0.01);治疗组神经恢复的感觉[(5.95±0.80)ms]及运动[(5.40±1.20)ms]电位的潜伏期时间均明显少于对照组[(11.10±1.55)ms,(9.92±1.40)ms],两组比较差异有统计学意义(t=20.84,17.33;P<0.01);治疗组感觉[(9.55±1.60)mV]及运动[(10.10±2.30)mV]神经电位波幅升高幅度均显著高于对照组[(5.05±1.70)mV,(5.77±1.50)mV],两组比较差异有统计学意义(t=13.63,11.15;P<0.01)。

结论

甲钴胺联合NGF对周围神经损伤的治疗效果满意,值得推广。

Objective

To explore the clinical effect of methylcobalamin in combination with rat nerve growth factor on the treatment of traumatic peripheral nerve injury.

Methods

One hundred cases of patients with peripheral nerve injury from Baise People′s Hospital were enrolled from January 2012 to January 2015.All patients were not suffering from other nervous system disease before the injury and were divided into the treatment group (a joint model NGF-methylcobalamin group, 50 cases) and control group (a-methylcobalamin group, 50 cases) by simple random grouping method.Pain, numbness symptoms improved and the improvement of neural electrophysiological changes were compared between the two groups 1 and 3 months after treatment.

Results

Compared with control group, pain, feel numbness symptoms of treatment group were significantly decreased, and pain symptoms effectiveness of the two groups were 56%(28/50), 94%(47/50) respectively, the difference was significant (χ2=19.25, P<0.01). Feel numb symptom effectiveness were 64%(32/50), 88%(44/50) respectively, the difference was significant (χ2=19.95, P<0.01). Incubation periods of sensory [(5.95±0.80)ms] and motor [(5.40±1.20)ms] nerve recovery potential of treatment group were significantly less than those of the control group [(11.10±1.55)ms, (9.92±1.40)ms], the differences were statistically significant (t=20.84, 17.33; P<0.01). Amplitudes of sensory [(9.55±1.60)mV] and motor [(10.10±2.30)mV] nerve potential of the treatment group were significantly higher than those of control group [(5.05±1.70)mV, (5.77±1.50)mV], the differences were statistically significant (t=13.63, 11.15; P<0.01).

Conclusion

The effect of methylcobalamin in combination with rat NGF on the treatment of traumatic peripheral nerve injury is satisfactory, it is worth promoting.
