众所周知,高血压是卒中的主要危险因素。国内外二级预防指南均建议通过严格控制血压以降低卒中发生风险,近年来随着24 h动态血压监测的广泛应用,血压变异性(blood pressure variability,BPV)对于卒中患者预后的影响越来越受到重视。但是迄今为止,缺血性卒中患者的最佳血压水平、血压昼夜节律以及晨峰血压与卒中预后的关系尚不清楚。笔者复习国内外文献的基础上对BPV的概念及分类、BPV的判断标准、病理性BPV的临床意义、BPV与缺血性卒中的关系、不同降压药物对BPV的影响进行综述。

It is well known that hypertension is a major risk factor for stroke.Secondary prevention guides at home and abroad are recommended by strict control of blood pressure to reduce the risk of stroke.In recent years, with the wide application of the 24-hour ambulatory blood pressure monitoring, the influence of blood pressure variability on the prognosis of patients with stroke was paid more attention.But so far, the best blood pressure level, blood pressure circadian rhythm and morning peak blood pressure with the prognosis of stroke in ischemic stroke pationes are unclear.The concept and classification of blood pressure variability, the judgment standard of blood pressure variability, and the clinical significance of pathological blood pressure variability, the relationship between blood pressure variation and ischemic stroke, the influence of different antihypertensive drugs on blood pressure variability are reviewed on the basis of literature at home and abroad.
