化脓性脑膜炎(PM)是儿童时期常见的神经系统感染性疾病,临床症状特异性低,若单纯依靠临床症状诊断,可能将导致适当的抗菌治疗发生延迟,实验室检查(包括外周血和脑脊液检查)可以提高早期诊断PM的能力。脑脊液(CSF)中乳酸水平升高,CSF葡萄糖/血糖比值降低以及聚合酶链反应(PCR)阳性是预测PM的独立因素。但是,部分患儿实验室检查结果亦不典型,各检验结果之间甚至相互冲突,因此,笔者主要对诊断PM的常见外周血和CSF检测指标作一综述,总结各指标诊断PM的效能,以指导临床早期诊断。

Purulent meningitis (PM) is a common neurological infectious disease with low specificity of clinical symptoms in children. If the diagnosis is solely based on clinical symptoms, it may lead to the delay of appropriate antimicrobial therapy. Laboratory tests, including peripheral blood and cerebrospinal fluid (CSF) tests can improve the ability of early diagnosis of PM. Elevated lactate level in CSF, decreased CSF glucose/blood glucose ratio, and positive PCR are independent predictors of PM. However, the laboratory examination results of some children are still atypical, and the test results are even conflict with each other. Therefore, the author mainly summarizes the common peripheral blood and CSF test indicators in the diagnosis of PM, and summarizes the efficacy of each indicator in the diagnosis of PM, so as to guide the early clinical diagnosis.
