The correlation of NF-κB p65 in blood monocytes with the severity of inflammation in acute exacerbation chronic obstructive pulmonary disease with palmatine treatments

Abstract:

Objective

To explore the value of NF-κB p65 activation level blood monocytes on evaluation of the effects of palmatine treatment in acute exacerbation chronic obstructive pulmonary disease (AECOPD).

Methods

In this clinical study, 100 patients with moderate AECOPD in Respiratory Department of the People′s Hospital of Fengjie County from October 2017 to January 2019 were randomly divided into two groups, control group (regular treatment) and observation group (palmatine and regular treatment), with 50 patients in each group. Before and after 7 days administrations, blood gas indexes (PaO2, PaCO2, and pH) and lung functions (FEV1% pred) were recorded, the serum levels of TNF-α and IL-8 were analyzed by ELISA, western blot was performed to measure phosph-NF-κB p65 and total NF-κB p65 in blood monocytes. Student′s t-test was obtained to analyze continuous variable between the two groups. Pearson correlation test was used to evaluate the correlation between the ratio of phosph-NF-κB p65/total NF-κB p65 and the serum levels of TNF-α and IL-8.

Results

After 7 days administration, the levels of pH, PaO2 and FEV1% pred [(7.42±1.87), (67.50±9.31)mmHg, (53.62±16.54)%] were higher than those before treatment [(7.16±2.31), (40.4±13.6)mmHg, (36.17±18.27)%], the levels of PaCO2, TNF-α, IL-8 and the ratio of phosph-NF-κB p65/total NF-κB p65 [(43.12±8.67)mmHg, (81.76±23.43)ng/L, (225.87±89.12)ng/L, (0.47±0.09)] were lower than those in the control group [(69.17±12.11)mmHg, (267.34±33.23)ng/L, (617.34±127.63)ng/L, (0.96±0.07)], the differences were statistically significant (t=-0.24, -0.31, -0.05, 0.04, 15.61, 25.12, 2.53, all P<0.05). At the same time, the levels of pH, PaO2 and FEV1% pred in the observation group were higher than those in the control group [(7.31±1.51), (62.71±14.12)mmHg, (44.64±10.47)%], and the levels of PaCO2, TNF-α, IL-8 and the ratio of phosph-NF-κB p65/total NF-κB p65 were lower than those in the control group [(53.64±10.25)mmHg, (114.82±25.96)ng/L, (362.65±106.97)ng/L, (0.68±0.11)], the differences between groups were statistically significant (t=-2.99, -3.07, -3.26, 4.62, 5.26, 7.27, 4.55, all P<0.05). In the observation group, the ratio of phosph-NF-κB p65/total NF-κB p65 was significantly positively correlated with the serum level of TNF-α (r=0.622, P<0.001) and IL-8 (r=0.571, P<0.001). No significant adverse reactions were found in the study period.

Conclusions

The activation level of NF-κB p65 in blood monocytes closely correlated with the inflammation level in patients with AECOPD. Palmatine is an effective and safe treatment for the patients with AECOPD possibly through inhibiting the activation of NF-κB p65 in the inflammatory cells.
