Abstract:
Objective To investigate the role of fallopian tube mucosal receptivity in the pathogenesis of tubal pregnancy.
Methods From January 2017 to January 2019 in Department of Obstetrics and Gynecology of Tongcheng People′s Hospital of Xianning City, wax blocks of oviduct tissues of 30 patients with tubal pregnancy were collected as study group, wax blocks of normal oviduct tissues of 30 patients removed by surgery for uterine and cervical cancer were collected as control group. Immunohisto-chemistry (IHC) staining was used to detect the protein positive rates of HOXA9, HOXA11, tumor necrosis factor (TNF-α), toll-likereceptor 4 (TLR4), estrogen receptor (ER) and progesterone receptor (PR) in the two groups. Real-time PCR was used to detect the expression levels of miRNA451 and miRNA223.
Results The positive rates of target proteins on tubal epithelial cells in the control group were: HOXA9 (13.33%, 4/30), HOXA11 (13.33%, 4/30), TNF-α (3.33%, 1/30), TLR4 (30.00%, 9/30), ER (56.67%, 17/30), PR (66.67%, 20/30). The positive rates of HOXA9 (76.67%, 23/30), HOXA11 (73.33%, 22/30), TNF-α (80.00%, 24/30), TLR4 (60.00%, 18/30) in the tubal epithelial cells of the study group were increased, while the positive rates of ER (16.67%, 5/30) and PR (13.33%, 4/30) were decreased. The positive rates of HOXA9(χ2=4.53, P=0.001), HOXA11(χ2=5.10, P=0.001), TNF-α(χ2=6.45, P=0.000), TLR4(χ2=4.22, P=0.005), ER(χ2=4.20, P=0.025) and PR (χ2=5.11, P=0.011) proteins between the two groups were statistically significant. The expression level of miRNA451 (98.59±8.83) and miRNA223 (115.28±15.20) in the study group were higher than those in the control group (50.25±8.96, 48.71±9.99), and the expression level of miRNA451 (t=6.20, P=0.001) and miRNA223 (t=9.26, P=0.000) in the two groups were statistically significant.
Conclusion During the pathogenesis of tubal pregnancy, the expressions of HOXA9 and HOXA11 in tubal epithelial cells are increased, which promotes the occurrence of tubal implantation. The expressions of ER and PR are down-regulated, which destroyed the protective barrier of tubal mucosa.
