    The relation between cortical epithelial fat thickness, CT value, Syntax score and coronary artery disease

    Objective

    To investigate the relationship between epicardial fat thickness, CT value, the scores of synergy between percutaneous coronary intervention with TAXUSTM and cardiac surgery (Syntax score) and coronary artery disease.

    Methods

    From February 2017 to October 2018, patients in Cardiology Department of Lanling People′s Hospital were selected. A total of 69 patients with coronary artery disease diagnosed by coronary angiography were enrolled in the study group, 60 persons with normal physical examination were selected as the control group.All cases underwent coronary CT angiography (CTA) examination, epicardial fat thickness and CT value were compared between the two groups. To compare the differences in epicardial fat thickness and CT values with different Syntax scores according to coronary angiography between the two groups.

    Results

    The levels of triglyceride [(1.62±0.68)mmol/L] and total cholesterol [(5.12±1.83)mmol/L] in the study group were significantly higher than those in the control group [(1.50±0.53), (4.90±1.20)mmol/L] (t=4.60, 3.82, all P<0.05). For the levels of high-density lipoprotein and low-density lipoprotein cholesterol, there were no significant differences between the study group [(1.49±0.43)mmol/L, (2.45±0.63)mmol/L] and the control group [(1.50±0.55)mmol/L, (2.50±0.55)mmol/L] (t=0.55, 0.34, all P>0.05). The epicardial fat thickness of the study group [(12.30±2.66)mm] was higher than that of the control group [(9.50±1.85)mm] (t=8.60, P<0.01). The epicardial fat CT value [(-98.60±15.66)HU] in the study group was higher than that in the control group [(-70.55±9.82)HU] (t=19.36, P<0.01). Epicardial fat thickness [(11.30±2.20)mm], epicardial fat CT value [(-90.60±9.55)HU] and the average Syntax score [(-17.32±8.42)point] in patients with low score group were lower than those in the high score group [(14.50±2.76)mm, (-115.30±13.36)HU, (-30.44±9.19) point], the differences were statistically significant (t=6.42, 9.58, 21.15, all P<0.01).

    Conclusion

    Epicardial fat thickness and CT value may affect the Syntax score of patients with coronary heart disease, which can be used as a reference index for disease evaluation, and it has a promotional value.

    Objective

    To investigate the clinical psychopsychological characteristics of patients with globus pharyngeus and the therapeutic effect of transcutaneous electrical acupoint stimulation(TEAS)at Shaoshang acupoint.

    Methods

    A total of 60 patients from September 2017 to June 2019 in the Otolaryngology and Gastroenterology Department of the Nansha Hospital of Guangzhou First People′s Hospital, the Guangdong Second Provincial General Hospital and the Affiliated Hospital of Guizhou Medical University were selected. The random number table method was used to divide the patients into control group (n=30) and Shaoshang acupoint group (n=30). With the treatment for 4 weeks, Shaoshang acupoint group was asked to stimulate Shaoshang acupoint with the current of appropriate strength, twice a day, 30 min each time, while control group was asked to stimulate non-meridian body surface position of 1-2 cm off the Shaoshang acupoint. The scores of Glasgow Edinburgh throat scale(GETS), Hamilton anxiety scale(HAMA), Hamilton depression scale (HAMD) and Pittsburgh sleep quality index (PSQI) were evaluated before and after 4 weeks treatment.

    Results

    Among the 60 patients with globus pharyngeus, 60.00% (36/60) were found to have sleep disorders, 65.00% (39/60) with anxiety, and 35.00% (21/60) with depression. After treatment, the scores of GETS, PSQI, HAMA and HAMD in the control group [(16.37±4.94), (12.37±7.51), (9.80±6.19), (10.67±7.63)] were compared with those before treatment [(20.50±5.26), (14.53±6.80), (12.63±6.53), (12.60±8.43)], the differences were statistically significant (t=3.98, 3.28, 4.59, 2.28, all P<0.05). The scores of GETS, PSQI, HAMA and HAMD 4 weeks after treatment of Shaoshang acupoint group [(12.43±3.79), (8.50±6.76), (9.07±6.52), (8.47±6.72)] were compared with those before treatment [(22.27±5.16), (13.43±7.30), (15.33±6.51), (13.97±4.99)], the differences were statistically significant (t=8.44, 6.93, 7.49, 5.77, all P<0.05). The decreased scores of GETS, PSQI, HAMA and HAMD in the Shaoshang acupoint group before and after treatment [(6.49±6.38), (3.44±3.85), (4.60±4.59), (3.47±5.22)] were greater than those in the control group [(2.93±4.04), (2.17±3.62), (2.83±3.38), (1.93±4.11)], and the differences were statistically significant (t=5.04, 2.80, 3.30, 2.94, all P<0.05). Meanwhile, the effective rates of treatment in Shaoshang acupoint group on GETS score [66.67%(20/30), 20.00%(6/30)], PSQI score [63.33%(19/30), 23.33%(7/30)], HAMA score [66.67%(20/30), 30.00%(9/30)], and HAMD score [60.00%(18/30), 26.67%(8/30)] were significantly higher than those of the control group, with statistically significant differences (χ2=13.30, 9.77, 7.28, 6.79, all P<0.05).

    Conclusions

    Globus pharyngeus are often accompanied with sleep disorders, anxiety and depression. Shaoshang acupoint treated by TEAS can effectively improve the symptoms, sleep disorders, anxiety and depression of globus pharyngeus.

    The application value of contrast enhanced ultrasound for detection of renal microvascular blood perfusion in diabetic nephropathies rabbits

    Objective

    To explore the value of contrast enhanced ultrasound (CEUS) for detection of renal microvascular blood perfusion in diabetic nephropathies (DN) rabbits.

    Methods

    Twenty rabbits were randomly divided into control group (n=10) and DN model building group (n=10) according to the random number table method. After successful modeling, there were 6 rabbits in DN group and 9 rabbits in control group. Real-time and low acoustic power imaging of the renal cortex was performed using CEUS with SonoVue to observe renal microcirculation. The time intensity curve (TIC) was used to obtain quantitative parameters including time to peak (TTP), peak intensity (PI), mean transit time (MTT), area under the curve (AUC) and so on. Renal function and morphology were also compared. Independent t-test was used to analyze measurement data.

    Results

    TIC revealed the TTP[(31.91±6.21)s, (22.30±8.05)s] and MTT[(58.68±9.82)s, (53.37±4.47)s] were longer in DN group than those in control group, with significant differences (t=6.66, 3.80, all P<0.05). Meanwhile DN group had a significantly low PI compared with control group [(29.31±7.73)dB , (34.45±5.46)dB, t=4.86, P<0.05]. However, no statistical significant differences were found in the change of other quantitative parameters including AUC [(2 442.65±725.47)dB/s, (2 255.31±209.09)dB/s, t=0.38, P>0.05], slope coefficient during wash-in (0.41±0.08, 0.37±0.06, t=1.26, P>0.05), slope coefficient during wash-out (0.02±0.01, 0.02±0.01, t=0.26, P>0.05) in both DN and control group.

    Conclusions

    CEUS may be useful for the early diagnosis of DN. TTP, MTT and PI may be useful quantitative parameters.

    Hypertensive disorders complication pregnancy (HDCP) is a common pregnancy-specific disease which is one of the leading causes of death in pregnant women and perinatal fetuses. Color Doppler ultrasound has been widely used in pregnancy examination, which is of great significance for monitoring the hemodynamic parameters of pregnant women and fetuses. Combined with two-dimensional ultrasonography, color Doppler ultrasound can reflect the blood flow state of fetus and pregnant women and the influence of blood perfusion of fetal organs. The maternal-infant hemodynamics change can reflect the change of pathological process of HDCP, and the uterus-placenta-fetal blood circulation in each stage of pregnancy, therefore it is of great significance in the forecast of HDCP and perinatal outcomes. This article reviews the value of color Doppler ultrasound on the changes of hemodynamic parameters in pregnant women and fetuses with HDCP.

    The value of spectral CT in the pretreatment evaluation of stereotactic radiotherapy for non-small cell lung cancer

    Objective

    To explore the value of Gemstone spectral CT in the evaluation of stereotactic radiotherapy pretreatment in patients with non-small cell lung cancer (NSCLC).

    Methods

    Fifty patients with NSCLC (34 cases of adenocarcinoma, 15 cases of squamous cell carcinoma, and 1case of large cell undifferentiated carcinoma) who underwent Gemstone spectral CT scan at the department of Medical Imaging of Shandong Provincial Qianfoshan Hospital from June 2015 to December 2017 were selected. The region-of-interest for a given spatial placement of the tumors was set. The iodine density in the arterial phase and the maximum diameter of the tumor were measured. T test for two independent samples was used to compare the differences of iodine density in tumors with different pathological types and different air contents (44 cases of solid tumors, 6 cases of ground-glass opacity). The cases were divided into 3 groups according to the tumor maximum diameter(d): d≤2 cm group (n=18), 2<d≤3 cm group (n=12) and d>3 cm group (n=20). The differences of iodine density between the three groups were compared and they were separately compared by one-way ANOVA. Spearman correlation was used to analyze the relationship between iodine density and tumor maximum diameter.

    Results

    There was no significant difference in the iodine density between adenocarcinoma and squamous cell carcinoma [(21.71±4.12)×100 mg/L, (21.29±3.93)×100 mg/L, t=0.24, P>0.05]. And there was no significant difference in the iodine density between solid tumors and ground-glass opacity[(19.81±6.89)×100 mg/L, (18.27±3.01)×100 mg/L, t=0.54, P>0.05]. The iodine density of d≤2 cm group, 2<d≤3 cm group and d>3 cm group were(23.12±5.35)×100 mg/L, (19.10±3.33)×100 mg/L, (13.90±5.99)×100 mg/L, respectively. The difference between three groups was statistically significant (F=14.62, P<0.01). With the size of tumor increased, the iodine density decreased significantly (r=-0.71, P<0.05).

    Conclusions

    With the tumor enlarges, the iodine density in the tumor decreases, which may be related to the relatively decreased blood volume of the tumor. The iodine density may be a non-invasive and quantitative assessment of the radio-resistance ascribable to the hypoxic cell population in a tumor, which may indicate the prognosis of radiotherapy to some extent.

    Evaluation value of transcranial electrical stimulation motor evoked potential in acute sciatic nerve injury model of rabbits

    Objective

    To explore the sensitivity of transcranial electrical stimulation motor evoked potential (TceMEP) after acute sciatic nerve root injury in rabbits and the evaluation value of its early waveform changes after acute sciatic nerve root injury.

    Methods

    Twelve New Zealand rabbits with 24 sciatic nerves were divided into 3 groups by random grouping method, one vascular clamping group (n=8), two vascular clamping groups (n=8) and the control group (n=8). The acute sciatic nerve injury model was established by using one or two vascular clamp compression of the sciatic nerve root, the TceMEP was recorded for 2 hours to observe the characteristics of TceMEP in the acute phase of sciatic nerve root compression injury, the percentage of TceMEP amplitude of stabilized waveform after compression injury to baseline amplitude and latency were recorded. The control group was used to exclude the effects of anesthetic and surgical operations on TceMEP. The lower limb motor function was evaluated using the Tarlov score after 2 days of clamping. Statistical analysis was performed using t-test, one-way ANOVA, rank sum test and spearman correlation analysis.

    Results

    TceMEP waveform of 1 vascular clamping group was changed at [0.20(0.00, 0.43)]min and was stable at [2.60(1.40, 3.35)]min; TceMEP waveform of 2 vascular clamping group was changed at [(0.35(0.05, 1.63)]min and was stable at [4.45(2.83, 5.65)]min. The percentage of postoperative amplitude when the waveform was stable to baseline in 1 and 2 vascular clamping group [(49.84±15.27)%, (15.47±12.64)%] were lower than the percentage of preoperative amplitude [(100.00±0.00)%, (100.00±0.00)%], there were statistically significant differences (t=9.293, 18.912, all P<0.01). There was statistically significant difference between the control group and the experimental groups in the percentage of postoperative amplitude when the waveform was stable to baseline (F=110.196, P<0.05). The postoperative latency when the waveform was stable in 1 and 2 vascular clamping groups [(12.60±0.61)ms, (14.29±1.24) ms] were higher than the preoperative latency [(12.21±0.59)ms, (12.31±1.10)ms], there were statistically significant differences (t=7.519, 5.721, all P<0.05). There was statistically significant difference of the postoprative latency in the control group and the experimental groups (F=6.702, P<0.05). The lower limb motor function scores after 2 days of clamping in 1 and 2 vascular clamping groups [4.00(3.00, 4.00), 2.50(2.00, 3.00)] were lower than the control group [5.00(5.00, 5.00)], there were statistically significant differences (Z=-3.664, -3.651, all P<0.05), which were also lower than the motor function scores before clamping [5.00(5.00, 5.00), 5.00(5.00, 5.00)], there were statistically significant differences (Z=-2.598, -2.585, all P<0.05). The percentage of postoperative amplitude when the waveform was stable to baseline and the Tarlov motor function scores after 2 days of clamping were significantly positively correlated in the control group and experimental groups (r=0.844, P<0.01).

    Conclusions

    TceMEP monitoring is very sensitive to the detection of acute sciatic nerve root injury, which is manifested as prolonged latency and decreased ampilitude, it provides the possibility of taking protective measures to prevent the occurrence of irreversible nerve root injury. The motor function of lower limbs can be predicted by the change of TceMEP waveform.

    Evaluation of clinical diagnostic value of a novel point of care testing for thyroid stimulating hormone

    Objective

    To evaluate the clinical efficacy of the latest thyroid stimulating hormone (TSH) point of care testing (POCT) technology-the "TSH flash card" by comparing with the TSH electrochemiluminescence quantitative detection technique.

    Methods

    Two hundred and forty cases who required for TSH detection from September 2014 to February 2015 in the Second Affiliated Hospital of Nanjing Medical University (n=120) and the Affiliated Zhongda Hospital of Southeast University (n=120) were enrolled. The tip of finger blood and venous blood samples of the subjects were collected for instant qualitative detection (TSH flash card) and quantitative detection of TSH level by electrochemiluminescence, in which the upper limit of normal value range detected by quantitative TSH method was converted to positive results, and the lower limit of normal value range or below normal value range was converted to negative results. Paired chi-square test and Kappa consistency test were respectively carried out to compare the difference and consistency of the two detection methods.

    Results

    A total of 122 positive results and 118 negative results of TSH flash cards were obtained. Simultaneously, there were 120 positive results and 120 negative results by electrochemiluminescence. There were 119 cases in which the two methods were positive and 4 cases were inconsistency. There was no statistical difference in the positive rate of the two methods (χ2=1.00, P>0.05). The positive consistency was 99.17% (95%CI: 97.54%-100.00%), the negative consistency was 97.50%(95%CI: 94.71%-100.00%) and the total consistency was 98.33%(95%CI: 96.71%-99.95%). The value of Kappa was 0.97 (95%CI: 0.93-1.00), showing good consistency between the two methods (χ2=224.33, P<0.01).

    Conclusions

    The TSH flash card method shows good consistency compared with the TSH electrochemiluminescence method. This method can be used for primary screening of hypothyroidism or subclinical hypothyroidism, and it can also provide timely monitoring of patients with hypothyroidism during the treatment of chronic disease, providing a new diagnostic tool for grassroots medical systems.

    Assessment of electroacupuncture and acupuncture on the gastric motility in critically ill patients by ultrasound

    Objective

    To investigate the effect of electroacupuncture and acupuncture on the gastric motility in critically ill patients by ultrasound.

    Methods

    Forty-five patients admitted to the ICU department of the first hospital of Jiaxing city from January 2015 to July 2016 were randomly divided into conventional group (CG, n=15), acupuncture group (AG, n=15), and electroacupuncture group (EAG, n=15), respectively.Acupuncture or EA was performed on bilateral Zusanli (ST-36), Shangjuxu (ST-37), Hegu (LI-4), and Quchi (LI-11) respectively.By using a single transverse section of gastric antrum with a Phillips ultrasound system, changes of antral contraction frequency (ACF) and antral contraction amplitude (ACA) were recorded and calculated.

    Results

    Compared to CG, ACF of AG and EAG at 1 h after water injection [CG(1.02±0.35)times/min, AG(1.33±0.44)times/min, EAG(1.83±0.40)times/min], 3 h[CG(0.95±0.51)times/min, AG(1.23±0.51)times/min, EAG(1.57±0.46)times/min]and 6 h [CG(0.88±0.39)times/min, AG(1.15±0.34)times/min, EAG(1.52±0.33)times/min]were significantly faster, the differences were statistically significant (F=15.79, 5.93, 12.33, all P<0.05). In AG, ACF at 1 h after water injection increased from (1.00±0.36)times/min to (1.33±0.44)times/min, the difference was statistically significant (t=6.30, P<0.05). In EAG, compared to the time point before water injection [(1.02±0.35) times/min], ACF at 1 h [(1.83±0.40)times/min], 3 h [(1.57±0.46)times/min]and 6 h [(1.52±0.33) times/min] after water injection increased significantly (t=36.60, 14.16, 17.22, all P<0.05).

    Conclusions

    Single transverse section of gastric antrum by ultrasound is an effective method to detect gastric motility.Acupuncture and EA can improve the gastric motility of critically ill patients, and that EA maintains a longer time.

    The variation of weight for age Z score in preterm infants with extrauterine growth restriction

    Objective

    To explore the variation of weight for age Z score in preterm infants with extrauterine growth restriction (EUGR)during the hospitalization.

    Methods

    Data of the premature infant from the Neonatal intensive care unit of Huai′an Maternity and Child Healthcare Hospital from December 1, 2010 to November 30, 2013 were collected retrospectively.The criteria for enrollment were gestational age <32 weeks, within the first 24 hours of life and survived when discharged, and hospitalization period ≥2 weeks.With the criteria, all of the premature infants were divided into extrauterine growth restriction (EUGR) group and non-EUGR group.And then, WAZ score count of each specific group was described to observe their growth situation during the hospitalization.

    Results

    Preterm infants with EUGR were 122 cases, the incidence of EUGR assessed by weight was 40.7%.WAZ scores of the EUGR group and non-EUGR group were almost negative. Z score of the EUGR group was lower than that of the control group.All of the WAZ scores were showed significant difference (Z=10.72, 11.00, 11.47, 10.72, 8.91, 14.42 respectively, P<0.05) between the EUGR and non-EUGR group in each observing time, such as at birth (-0.83±0.37, -0.25±0.39), the 1st week (-1.16±0.35, -0.54±0.43), the 2nd week (-1.14±0.35, -0.46±0.46), the 3rd week (-1.21±0.34, -0.58±0.39), the 4th week (-1.35±0.41, -0.65±0.42), and at the discharged time(-1.68±0.38, -0.52±0.44).

    Conclusions

    The weight for age Z score in preterm infants with EUGR show a progressive decline.The WAZ score′ variation tendency have some clinical significance to prevent EUGR.

    The application of assessment of blast traumatic brain injury EEG power spectral analysis

    Objective

    To determine whether EEG spectral analysis could be used for the analysis of animal models with blast traumatic brain injury.

    Methods

    We built animal traumatic model using TNT bomb without fragments.All the 15 goats were divided into 3 groups according their brain pathology change.We recorded EEG from the animals at the time before bomb exploring as well as 3 hours after blast.Then, we assessed EEG spectral differences from 1 to 30 Hz and compared EEG spectral with different groups, following with alculating the absolute power value in each band and (δ+ θ)/(α+ β) value.

    Results

    The injury group′s spectral power values with 3 Hz、4 Hz、5 Hz and 6 Hz were (6.55±2.82)μV、(4.71±0.93)μV、(3.47±0.76)μV and (2.82±0.54)μV.The healthy control group′s spectral power values with 3 Hz、4 Hz、5 Hz and 6 Hz were (3.20±0.58)μV、(3.16±0.40)μV、(2.66±0.34)μV and (2.15±0.10)μV.It′s of significant difference between the two groups, the P value was 0.031, 0.009, 0.049 and 0.026. The healthy control group′s spectral value of δ band was (3.20±0.99)μV and θ band was (2.27±0.64)μV and (δ+ θ)/(α+ β) was (1.35±0.46)μV.The injury group′s spectral value of δ band was (9.81±1.06)μV and θ band was (4.97±1.41)μV and (δ+ θ)/(α+ β) was (3.19±0.74)μV, which were significantly higher than the healthy control group(P=0.045, 0.048, 0.024). The no injury group′s spectral value of δ band was (3.01±0.93)μV and θ band was (2.31±0.80)μV and (δ+ θ)/(α+ β) was (1.47±0.32)μV, which were similar with the healthy control group (P=0.794, 0.940, 0.704).

    Conclusion

    EEG power spectral analysis can be used as a flexible tool to demonstrate the animal model with traumatic brain injury.

    Mini clinical evaluation exercise (Mini-CEX) is a mini assessment method which can be repeatedly implemented on outpatient and inpatient at any time. With the advantages of high feasibility, it is demonstrated to be a valid and reliable method.The application of Mini-CEX gives more chances in clinical practice, which is helpful in enhancing the clinical comprehensive ability.Mini-CEX has a timely information feedback from the both participators, it can promote the teaching and learning, Mini-CEX will play an important role in medical education.

    Evaluation of subclinical dysfunction of left ventricle induced by anthracyclines in patients with diffuse large B-cell lymphoma by real-time three dimensional echocardiography

    Objective

    To explore the value of real-time three-dimensional echocardiography (RT3DE) in the assessment of subclinical dysfunction of left ventricle (LV) induced by anthracyclines in patients with diffuse large B-cell lymphoma (DLBCL). Furthermore, to explore the sensitive parameters of RT3DE in the assessment of subclinical dysfunction of LV.

    Methods

    RT3DE images were acquired from twenty-six patients with DLBCL before chemstherapy, after 2(100 mg/m2), 4(200 mg/m2) and 6(300 mg/m2) chemotherapy cycles for evaluation and analysis to obtain LV end-diastolic volume (EDV), end-systolic volume (ESV), systolic volume (SV) and ejection fraction (EF). Global longitudinal strain (GLS), global circumferential strain (GCS), left ventricular twist (LVtw) and left ventricular torsion (LVtor) were also calculated.

    Results

    Changes in EDV[(71.4±14.8)mL, (76.3±15.6)mL, (81.7±19.5)mL, (79.7±16.2)mL]、ESV[(22.9±5.1)mL, (25.1±5.4)mL, (27.1±8.1)mL, (26.6±6.7)mL], SV[(48.6±10.4)mL, (51.2±11.0)mL, (54.6±12.8)mL, (53.1±10.7)mL], EF[(67.9±3.1)%, (66.9±3.1)%, ( 67.0±4.1)%, (66.7±3.6)%], GLS[(-26.9±4.8)%, (-27.1±4.4)%, (-25.5±7.5)%, (-26.7±7.1)%], GCS[(-34.7±2.5)%, (-33.7±3.4)%, (-34.1±3.8)%, (-33.7±3.3)%] showed no significant degradation compared with the parameters at baseline as well as, after 2, 4 and 6 chemotherapy cycles (F=1.095, 2.297, 1.388, 0.628, 0.344, 0.585, respectively, all P>0.05), while the variations on LVtw[(19.9±6.8)°, (13.1±10.0)°, (13.5±8.1)°, (11.1±5.8)°], LVtor[(2.9±1.2)°/cm, (1.8±1.4)°/cm, (1.8±1.1)°/cm, (1.5±0.8)°/cm] decreased statistically compared with the data at baseline, and after 2, 4 and 6 chemotherapy cycles (F=6.007, 7.013, respectively, P<0.05, respectively). Changes on LVtw, LVtor showed statistically reduction after 2, 4 and 6 chemotherapy cycles compared with the parameter at baseline (LVtw: P=0.025, 0.016, 0.001, respectively; LVtor: P=0.008, 0.007, 0.000, respectively).

    Conclusion

    LVtw and LVtor derived from RT3DE may be used as sensitive parameters to detect subclinical left ventricular dysfunction in patients with DLBCL underwent anthracycline therapy.

    Alzheimer disease(AD) is the most common cause of dementia, and it has caused a huge economic burden to our country and other nations.Therefore, how to carry out an effective prevention and treatment strategy in the early stage of the disease, has become the key of the treatment of AD.In 1984, the National Institute of Neurological and Communicative Disorders and Stroke, and the Alzheimer Disease and Related Disorders Association (NINCDS-ADRDA) firstly announced.With the development of our understanding of AD as well as the new technologies coming out, the diagnostic criteria also develope significantly, such as the more emphasis on mild cognitive impairment(MCI) stage and putting the biomarkers into the diagnostic criteria.However, there are significant differences among diverse standards(e.g.IGW, NIA-AA, etc), so we summized this article reviewed the changes of the AD diagnostic criteria, and gave a detail discussion about them.

    In the early 1960s, the traditional concept of disease and the dichotomy of function/organ within biological medical model were criticized by Engle.However, there is little awareness of the limitation of making organic disease the primary focus of clinical medicine and medical research.It is not that certain disorders lack organic explanations, but our assessment is inadequate in most clinical encounters.Bio-psycho-social medical model has been at theoretical level and been far from clinical practice for lacking of operational strategies or methods.The primary aim of psychosomatic medicine is just the incorporation of its operational strategies with clinical practice.Psychosomatic model will offer unprecedented possibility for the identification and treatment of various difficult medical problems.Taking full advantage of clinimetrics methods may be a good remedy for biological model and improve patient′s clinical course, final outcome and the quality of life.

    Objective

    To explore the value of high-flux hemodialysis in improving left ventricular function in patients with uremia evaluated by ultrasound.

    Methods

    Sixty-seven patients with uremia who had been treated by maintenance hemodialysis were selected and divided into two groups according to the dialysis modality: conventional hemodialysis(HD)group(forty-four cases) and HFHD group(twenty-three cases). Urea nitrogen (BUN), creatinine(SCr), serum calcium(Ca), serum phosphorus(P), uric acid(UA), albumin(ALB), parathyroid hormone(PTH), tumor necrosis factor alpha(TNF-α), interleukin-6(IL-6), endothelin-1(ET-1) were detected in the two groups before treatment and six months after treatment, and left ventricular end-diastolic posterior wall thickness(LVPW), left ventricular ejection fraction (LVEF), mitral E/A value, left ventricular peak systolic mitral annular displacement (MAPSE) were measured by ultrasonography.The statistical method unpaired t test was used to analyse the data.

    Result

    There were no significant differences in the decreasing rates of BUN, SCr, UA, IL-6 between the HD group[(6.50±2.51)%, (-16.94±8.13)%, (-2.50±0.98)%, (-0.31±0.12)%, respectively)] and the HFHD group[(-7.81±2.07)%, (-15.33±6.21)%, (-3.16±0.53)%, (-0.33±0.18)%, respectively](t=-1.984, -0.830, 1.684, 0.543; P>0.05) after treatment, there was significant difference in the decreasing rate of serum P between the two groups [(-5.23±2.12)%, (-11.53±4.43)%, t=-7.896, P<0.05]. The decline rates of PTH, TNF-α, ET-1 of the HD group[(9.62±3.55)%, (16.31±6.83)%, (21.21±8.13)%, respectively)]were lower than those of the HFHD group [(-22.13±7.33)%, (-19.73±7.25)%, (-31.32±12.13)%, respectively)](t=-23.960, -20.081, -21.110; P<0.05). The thickening rate of LVPW of the HD group[(0.98±2.31)%] had no significant difference with the HFHD group[(0.43±0.17) %] (t=1.136, P>0.05). The increasing rates of E/A, LVEF, MAPSE of the HD group[(1.42±1.16)%, (2.75±0.97)%, (2.17±0.67)%, respectively)] were lower than those of the HFHD group[(9.02±2.74)%, (11.23±3.45)%, (17.6±4.44)%, respectively) ](t=-15.946, -15.282, -22.716; P<0.05).

    Conclusion

    Compared with HD, HFHD can improve the left ventricular function of patients with uremia.

    Objective

    To explore the noninavasive prenatal genetic testing technology in the role of screening for chromosome aneuploidy.

    Methods

    Massively parallel sequencing was applied to analyze fetal chromosome sequence copy numbers in seventy-four pregnant women with clinical indications for prenatal diagnosis.According to the different clinical indications, four groups were divided.The rate of high risk of aneuploidy in each group was counted.High risk of aneuploidy was showed in eleven cases; fetal karyotyping was also carried out on amniocentesis samples.

    Results

    The rate of high risk of fetal chromosome aneuploidy was 10.5%(4/38) in advanced maternal age group, 17.4%(4/23) in the increasing-risk maternal triple-marker screening test group, 30.0% (3/10) in the abnormal ultrasound finding group, 0.0% (0/1) for family history of chromosomal abnormality.The total rate of high risk of aneuploidy in all samples was 14.9% (11/74). In eleven high risk cases of noninvasive prenatal testing, the results of noninvasive prenatal testing of nine cases were accordant with fetal karyotyping, but two cases were false positive.

    Conclusion

    Noninvasive prenatal testing is an effective method for screening fetal aneuploidy.It has a broad clinical application prospect.

    Incidence of intra-abdominal hypertension and its risk foctors in critically ill patients

    Objective

    To discuss the incidence of intra-abdominal hypertension (IAH) and its risk factors in critically ill patients.

    Methods

    Prospective cohort study was carried out in our general intensive care unit (ICU). A total of fifty-four patients were enrolled between July 2013 and November 2013.Intra-abdominal pressure (IAP) was measured though the urinary bladder catheter indirectly.Indicators including demographics, main diagnosis on admission, mechanically ventilated and the PEEP (positive end-expiratory pressure), laparotomy, clinical test results, CVP, cumulative fluid balance, severity scores and so on were recorded.Logistic regression analysis was used to investigate risk factors of IAH.Risk factors of IAH were analyzed by logistic regression.

    Results

    In our study, the incidence of IAH in critical patients was 37.0%(20/54), including ten persons in Grade Ⅰ, eight persons in Grade Ⅱ, two persons in Grade Ⅲ, none in Grade Ⅳ.Higher acute physiology and chronic health scores (APACHE-Ⅱ) [(20.1±6.2) scores, (14.9±5.6) scores, t=8.04, P<0.05], Sequential organ failure assessment (SOFA) scores [(10.4±6.4)scores, (5.5±4.3) scores, t=7.31, P<0.05], central venous pressure[ (10.4±2.8)mmHg, (8.9±2.0)mmHg, t=6.21, P<0.05)]were measured in patients with IAH than those in patients without IAH.Risk factors associated with IAH were mechanical ventilation, ascites, septic shock and disorders of biliary system(P<0.05).

    Conclusions

    The high incidence of IAH is observed in ICU critical patients, mainly in mild and moderate degree.And a close correlation is found between IAH and its risk factors including mechanical ventilatia ascites, septic shock and disorders of biliary system.

    Aortic valve replacement (AVR) is generally accepted as the standard operation to treat symptomatic aortic valvular diseases now.AVR can apparently improve the clinical symptoms and hemodynamics of patients with aortic valve stenosis or regurgitation.AVR has good effect and a higher long-term survival rate. Although transcatheter aortic valve implantation (TAVI) has also been accepted as an effective means to treat aortic valvular diseases, AVR is still the most reliable surgical procedure.Echocardiography is the most important method to diagnose aortic valve stenosis or regurgitation.Echocardiography is also a safe and reliable non-invasive detecting technique, which has important value on having preoperative evaluation, determining operation indication, choosing operation procedure and judging prognosis.The early complications of AVR affect the prognosis of patients after the surgery.Echocardiography is the most widely used clinical methodology for detecting the early complications that occurred within 30 days after the surgical procedure of AVR.Conventional echocardiography is most widely used, but new echocardiographic techniques such as tissue Doppler imaging(TDI), two-dimensional or three-dimensional speckle tracking imaging (2D or 3D-STI) and stress echocardiography are getting more and more attentive in recent years.However, there isn′t enough sufficient evidence to prove the predictive value of echocardiography on early complications after AVR.This article reviewed echocardiographic detective methods of main early complications after AVR, which may provide evidence to study the predictive value of preoperative echocardiographic index on early postoperative complications.

    Preliminary clinical application of three-dimensional cephalometric in orthodontics

    Objective

    To evaluate the clinical application of 3D cephalometric in orthodontics and compare conventional cephalometric with 3D cephalometric, which reconstructed by cone beam computed tomography (CBCT) images.

    Methods

    From May 2012 to December 2012, there were 13 patients going with orthodontic therapy, among which 5 patients with severe dental maxillofacial malocclusion and 8 patients with impacted tooth. All patients were detected by regular pantomography and lateral cephalogram, then CBCT scans with consent. All patients were detected by 3D cephalometric analytics with professional software compared with conventional cephalometric.

    Results

    3D constructed CBCT form showed more accurate images.Compared with 2D, 3D cephalometric measurements showed differences in ANB, MP-FH, L1-NB, SNA, U1-NA parameters(t=2.63, 2.59, 2.68, 3.08, 3.21; P<0.05), there was no significant difference between other parameters.(t=1.32, 1.17, 1.46, 1.37, 1.62, 1.03; P>0.05).

    Conclusions

    CBCT combined with 3D cephalometric takes great advantage to conventional cephalometric. It has important clinical guidance to establish treatment scheme for patients with dental maxillofacial deformity.
